Abstract
Based upon a transcript of an audio recording of an actual counselling interview between a nurse-counsellor and a mastectomy patient, this paper examines the ways in which the parties to a formal organizational episode are involved, in and through their talk, in describing that organization to themselves. Such descriptions are part and parcel of doing the work involved in this very organizational episode. That is, given that the organizational concern here is to explicate, assess and monitor the patient's emotional reactions, the interlocutors achieve this end by locating these emotions within descriptions of the routine workings of the selfsame organization. The fact, then, that this session describes the organizational diagnosis and treatment of breast cancer from within that process means that the session can be regarded as a classic case of a reflexive phenomenon. The counselling talk is a constituent feature of the circumstances it describes and is in turn elaborated by them.
